IMPORTANT 2026 UPDATE:
High Point Clinic is currently CLOSED
As of Saturday, September 26, 2026, High Point Clinic will no longer be seeing patients. We have assisted the transfer of all of our patients to new providers as needed and hope for continued success for all those who we have had the privilege to assist in their journey of recovery. We have provided our updated resource list HERE for those who continue to benefit from this information.
We request that you to utilize the Release of Information provided to you from High Point Clinic in order to request specific patient records. Please be specific with both dates and types of documents requested and be mindful that 42 CRF Part 2 must be recognized when requesting medical records which are requested for any substance abuse patient. You may fax your filled out and signed request to 423-631-0732. Please allow 15-30 business days for all medical record fax requests to be processed.
